I've been getting to grip with WiX recently (http://wix.sourceforge.net/) which is the "Windows Installer XML (WiX) toolset" which makes it easy to create an MSI installer for your game just by using notepad to edit an XML document that defines your game installation.

It works pretty well as you can see by my updated Blocks Away installer - see showcase forum entry http://www.underwaredesign.com/forums/index.php?topic=2368.0 for details.

The WiX site on sourceforge (Microsoft produced this utility and released it as open source - hoorah) provides some pre-compiled binaries which is what I used and it is fairly simple to take and amend some of the example installation XML files that they have to produce a working MSI installer for your game.
